Lost everything because of early termination

Yesterday the server crashed or something and I lost everything because of this. 
I got killed in action but I did not get shot or whatsoever and I never left the map.

 

What is this and why don't I get my stuff back since I insured everything.

You get stuff back from insurance if it was not brought out of raid by a Scav or PMC
I've never been KIA so wouldn't be able to tell you how it happened.

You may have crashed but other players or scavs could of still been alive and killed you. On their screen all they would of seen is an AFK PMC and to them it's an easy and free kill.
